Do you mean "extract" rather than eject? If you mean extract then a little more information would be helpful.
Any markings on the fired and extracted cases? I have seen the issue you are having caused by a rough chamber, is it possible to get your hands on a borescope to look at the chamber?
You say the brass is new Lapua and that it has not been an issue until the third firing, but in reading what you posted above my next question is this - have you been using the same brass in both barrels?
If so then it is probably an issue with one barrels chamber being slightly larger than the other in the base where the brass is not being resized.
I have seen this happen in more than one instance and it is not an uncommon problem.
