2 in 1 out groups

Is there a common cause for this type of grouping where you can have 2 holes touching and the third is out in left field?

Three shots is hardly a fair test and can't be used as a basis for judging the accuracy or potential of a given load. You need to shoot a minimum of 5-shot groups, 10-shot would be even better. If you take one of those 2 and 1 groups and shoot additional shots you might find that the 2 in the same hole are the real flyers.

As a general rule, a multi-shot group with a single flyer means that the rifle is trying to tell you something. It's saying,'I can shoot but you can't.'

So... lets take the OPs example a little further.

Lets say we have a 6 Dasher that is shooting decent groups @ 300yds - but with a fairly regular 'dropped' shot, i.e. 3-4 shots touching, but one out low about 3/4-1'. One or twice, sure statistical fluke. Couple more times, start paying closer attention to what I may be doing wrong in terms of gun handling on the bags. Few more groups and I start going through my loading procedures trying to figure out where things may be going astray. By then, it's not a 'fluke', but I'll be dipped if I can figure out whats causing it.

Well, really hate to admit that I've noticed my shooting seems to open up the older I get. Was hoping those goofy groups were caused by something else than me. You guys are brutal.,Grin)

A group with one flyer may indicate an OAL problem with the flyer. I sorting bullets I regularly find 3-4 that are out of tolerance with the others by a much as .002-.003in which will usually make the loaded OAL either shorter or longer than the others and will effect the impact.
