Split Bushing

I've been reloading for a good while now and this is the first Redding bushing that split, I have over 36 bushings. I was full length sizing some of my 308 Lapua brass and noticed my bullets did not feel right when seating them, the bullets went in too easy and I was able to pull them out by hand. This is when I discovered the split in the bushing. I was using a .340 neck chamber and my brass necks were turned to approximately .014" thickness, just to clean them up. I have been using the 334 bushing for several years. Has anyone else experienced this?
 
No splits. Was outside of case neck lubed, Before sizing.
Steel or titanium nitride bushing?

Looks like a defect in the bushing.

Google AI- Redding bushings, used in Type S neck and full-length sizing dies, are heat-treated steel, typically hardened to Rc 60-62. While rare, a split Redding bushing, as noted in user reports, is usually caused by extreme stress during the resizing process, often from improper lubrication or significantly undersized bushings, resulting in a failed component rather than a designed "split" feature.

I have been using Imperial Application Media with Dry Neck Lube on the necks and Imperial Sizing Die Wax on the body for years with all my cases. I'm thinking a defective bushing also.

I have never seem a bushing split but I have seen several sizing dies split.
