Look to see if theres rifling in the barrel first thing
This is what i see. Date not set on bore scope, pictures taken 30 min ago. Shot of the lands and groves and a shot of the throat (i think).

Looks good to me !

When my pistol bullets key hole it means more powder needed.
I've not had any rifle bullets do it.
 
Agree , looks ok.. could as has been said just a crap bullet.... If it doesn't happen again , don't worry just keep on shooting...
 
View attachment 1072410 Factory savage 204 ruger, unknown round count as I have put 200 rounds through it since I got View attachment 1072409 on a trade about a year ago. Has shoot good, mounted new scope today, put target at 50 yd s to get on target. First shot on a clean barrel with proven load keyholed. Checked bore, brass every thing look good, adjusted scope from the key hole, shot the two on the right. Went to 100 yd s everything seems normal. Better then 1/2 in groups. Any idea what happened?

RetiredArmy looks like you just had one bad bullet to me. I had the same happen on my 6.5 creedmoor at around 400 rounds of Hornady 147 eld m down the barrel but just that one out of 2400 now. It still shoots 1/2 MOA even with the rifling burnt out to .112 over new. It's a Savage 12FV heavy barrel, great gun, good luck.
