Have you seen this before

Proceed with caution. I bet whatever caused the issue is going to happen again. Looks like the brass flowed. Pin is the most logical. I suggest measuring the flash hole on the bunch and see if those are on the small side.

How many times fired? Wet tumble? What made you look?
 
.308 Lapua Palma brass with a raised lip around the flash hole, out of 100 case 2 had this……
When you say 2 out of 100, they came from factory this way? I'm thinking, if you had an oversized decapping pin, it would have affected other cases in that run as well. If they arrived that way, discard them. Propably mfg mishap.
 
It is a hot load for the rifle it was fired in.
Did you do ‘anything’ to the flash hole?
 
Lapua makes the flash hole in their small primer 308 brass .059.Most decapping pins are .065.They claim that the smaller hole makes a longer tongue of flame which helps with consistent ignition.Turn the pin in your die down to .057 and you'll be ok.

Judging by that ejector mark thats a hot potato. Dont think that did your mushroom though. Its getting hotter outside so watch that load

It may be even spicier now that it’s conformed to the chamber a bit.

I’ve seen that in cheaper brass from the punching primer pocket processing.
 
It appears to be "Material Drag" from the Die punch . As a Die punch begins to wear at the edges , it will push material down , instead of cutting through cleanly , and the Guide-hole in the base of the Stamping Die begins to wear , similar to a F/P Bushing . The end result is what you're seeing in the photos . Almost make a wager those cases had that same "lip" before they were ever fired . Lapua does make mistakes , but not very often . That's why there were only two out of 100 . Guessing the operator caught it , pulled the stamping Die , and had it sharpened , or there would have been more got out in the Lot .
