My money is on the crooked barrel. Even if the scope bases, holes in action, or recoil lug was thicker on one side I think looking down the bore and adjusting the scope should get really close to being on target. Just a few thousandths of an inch bend near the muzzle would be undetectable by the naked eye but will throw the bullet way off.
I'd bet that a boresighting tool like Bushnells with the arbor that fits inside the bore would tell the tale.
