What type rifle, bolt or AR? Sounds like there is something floating around loose in your bolt locking lug area and getting into the shoulder area when chambering a round . Anything big enough to make that gouge should be easily visible. Clean your action. Take a good light and look in the bolt area. Turn gun over and shake it and see what falls out.

The Nick appears to be made during insertion, is there a sight screw protruding through the receiver, is the rifle AR10?

One other thought, I had a similar problem with an AR in 6mm Grendel where the base expanded and could not be resized until I purchased a small base die. Obviously a small base die is not an option, however that is a dimension that you might check.

I have looked but don’t see anything protruding. This is from a semi-custom bolt gun.
