Different primer or seating depth is were I would be looking.
If those don't fix it different powder as long as its not mechanical.

Gun handling. Front bag poor fit to forend. Rear bag that's convexed on the bottom and rocks left and right. Trigger hand pressure on the stock grip. Shoulder pressure into the butt of the stock. Cheek pressure on comb. ANY AND ALL OF THESE GUN HANDLING MISTAKES can make a rifle do what you're seeing. That being said, if I had to put my finger on one thing I would say the bullets are the culprit.

Trying to find a new bullet for my .20 TACTICAL. This has happened several times with a 40 grain V max. I’ll shoot 2-3 bullets through the same hole and one bullet will be 1/2”-3/4” off that hole. Is there something I’m missing or it can just be a list of things? I’m sure some of it can be the loose nut behind the wheel (the shooter) but I have a bunch of targets like this. Most of the time it’s the 40 grain V Max.

for an easy button check try some bergers or better bullets like regina said. that happens most of the time for me with hornadys. lately its been with 6mm 87 vmax. with the cost of components these days i dont think ill use them with hornadys anymore.

A friend had a total custom long range hunting rifle chambered in a short magnum, if memory serves. Targetish stock with adjustable cheek piece, heavy enough barrel. He was a meticulous reloader and excellent test shooter of his magnums, but this one seemed to shoot consistent two and ones. This is what cured the problem. I had him move his front rest back quite a lot so that the bag was only a couple of inches in front of the action. He had the cheek piece cranked up so that he had hard contact with his cheek on the stock. I had him lower the cheek piece to the point where it barely grazed hie cheek, and pull the rifle moderately into his shoulder using the second and third fingers of his trigger hand on the relatively vertical pistol grip. Problem solved. You would be amazed at the number of fellows that I have been able to help, reducing groups by 2X or more with stuff like this. You might be a lot less surprised at the number of guys who ask serious accuracy questions who have not had their action bedding attended to properly, and who resist the idea of putting out something, however modest, so that they can judge the wind between the bench and their target. Most guyte who have shot a rifle for a couple of decades thinks that they are shooting correctly, and that some little reloading trick is all that he would need to solve any accuracy problem. I wish that it was that simple. In your case it may be. Try other bullets.

According to Jack Neary (as referenced in the video linked below) if you're getting 4 + 1 flyer it may mean your charge is just a little too hot. Three plus two flyers means it's even more too-hot.

I don't have enough experience to know if this is the case in your situation but I thought I would put it out there since no one else has mentioned it as a possibility.
