Stuck case in the chamber

I keep a small flashlight on the bench with an attached note reminding myself to check powder fill on every case before seating bullets.As careful as I think I am and try to be,I have found several out of the last 400 rounds that I missed charging and only caught it because of the flashlight.
I do this as well, and scan each row, row by row. I've never had a squib doing this. But using a multi-station loading setup like a Dillon, etc., can get guys in trouble if not 100% focused. Ask me why I know....
 
I personally haven’t seen a primer lodge a bullet far enough into the rifling to allow another round to chamber. I’ve had a couple in my ppc that I accidentally didn’t put powder in and never moved the bullet. I’m guessing pistol powder or an extremely low powder charge.
Long throat , short bullet (fireforming), light neck tension Planets could align and create a situation where it could occur. I agree however I would not think it would have enough space to allow the bolt to close

How are you doing powder charges? Using a powder throw, or actually weighing each charge on a scale? Here’s why I ask this. I’ve had powder bridge in a powder thrower at times, that it doesn’t all drop out into a case. Many years ago, I was loading some rounds just throwing and not weighing. Suddenly I had a case get over filled, and the prior case was only half full. I learned to check every case with a flashlight before seating a bullet. I also quit loading straight from a powder throw from there on.
Same here, powder clogs/bridges scared the poo outta me and went to weighing each and every charge until I got a Chargemaster. Immediately tweaked it with a dispensing tube bush to much reduce the overthrow inaccuracies reported they do.
 
As far as getting the bolt open I remember a post about this IIRC it involved taking a block of wood and a hammer and knocking the back end of the bolt towards the muzzle What you want to do is push the ruptured case forward a little and create a little space then attempt to open the bolt
Will that work if your bolt lugs are locked up against the action lugs?
 
How can the bolt move forward with the action lugs blocking it? Would the also mean the bolt moves back with pressure and increase the headspace without lifting the handle??

Not trying to be argumentative here .... just curious. How far can it move? What kind of tolerance is there between action and bolt lugs when locked up?

Whatever the headspace is there is that much movement.

No - the bolt does not move backward when fired, as there should be zero clearance when the action is locked up, thus no additional headspace caused by that when the gun if fired. As far as the bolt moving forward when smacked from the rear, I won't comment on that. I'm not familiar with the Defiance action.
