Strange 12 ga fired hull

This was a factory Nobles 12 ga low base 7.5. Shotgun is an older gas operated (12 ga). Cleaned assembled to test fire before wood refinish. Round sounded different (2 distinct pops) action partly open when the second pop and flash. Hand finished the ejection, barrel was clear. Several more rounds, all functioned perfectly. Low base was still attached, but came apart after handling, plastic body is as extracted. This is a first. IMG_20230321_113036042.jpg very little recoil, or noise.

I had something similar happen same as you 2 times one right after the other same box of reloads I loaded my self, never had a problem before or after, 2 pops each and failed to eject I could see the shot exit the barrel
 
Yes, did not eject. Very low recoil, second pop occurred with bolt open slightly. That's when "flash" out of the receiver occurred. Pops were very close together, but recognisable. No damage to shotgun, or flesh.
 
I used to shoot 10000 rounds a year at skeet. Occasionally you would get a hang fire, half second delay. Can't imagine how enough energy happened to partially open an action. Im thinking partial case(shell) head seperation inside chamber .
