9-10k South Bend DTS-100K Double Tool or production cross slide assembly with all of the hardware shown
Includes both the rack gears (grey casting with handle and grey casting that bolts to lathe) as well as cross feed nut (so you can use this just your normal cross slide) you must unbolt the hand lever casting to use with your screw.

that is an interesting piece of hardware , you might post it over on the practicalmachinist.com Southbend forum. I have a heavy 10 so it would not work on mine. Jim
