Have 250 of the M18s (320 carry model) with the units I provide training to and there hasn't been one case of an accidental or negligent discharge.

If it happened on video, in a holster as the lawyer said (think about that for a minute Lol) there is not much to say. Makes me wonder if that dept received a shipmemt of guns at the same time with the same defect. I used to see that occasionally in working as a mechanic at a auto dealership where we would see high failure rates of a certain part then never see one again. I suppose that could happen with a mass produced firearm as well.
