I have never lost a 5.56 going to 300W or 300AAC. Most of the time I am sorting thru my brass and pulling the ones that have the dings in the neck or high in the shoulder as that part gets cut off. I save my best brass for the 223.
Dillon spray lube on a couple hundred cases in an old cake pan, RCBS dies, shell holder, and Rockchucker. I size them as fast as I can get them in and out. Sizing die, decap stem only. Once they have been sized and decapped, they go into the saw to have top cut off, then trim, debur, and run primer pockets as necessary. There are only a few things that can cause the rim to get pulled off. Loose shell holder would not be first on my list of things to check.....unless it is just plain junk.....
I would be looking at your lube, die, and sizing process....shell holder is pointing to a different root cause of your problem.....
