Score Cards

I'm looking for a PDF or something of a match score card (20 shots + SS). I thought Desert Sharpshooters used to have a section with plot sheets and scorecards, but it looks like they took it down, or maybe moved it.

Anyone have a score card handy (name, classification, 20 shots record + SS, etc...) they can post or send me?
