Bullet Failure Question

As many of you know we are working on the bullets failure to reach the target situation. A condition has presented itself for which we need more data. For the purpose of clarity, I ask that you refrain from providing information,answers) that are not specific to the question. The question is: If you personally have experienced bullet failure can you please list the cartridge, powder,brand and type is most important), load,amount of powder in grains), primer and bullet?

We have clearly identified that two separate results are occurring. One result is a condition that produces failure of any bullet brand. The other is a condition that produces the failure of one brand but not another under the same conditions. My goal here is not to suggest that the powder is the source of one or both of these results but rather to gather more data. Your feedback is helping us draw closer to solutions on this situation. While I have heard of such I can only relate to ONE in the many, many bullets I have shot. It was a green bullet of 175gr weight driven with 45.0gr Varget, lit with a Russian KVB-7 lot #,1)speed was 2720 fps. I think that the hollow point had come apart as I heard a BUZZ as it left the bore, THAT bullet would up as a 6 ring hit @7 at 1K, and yes it was frustrating.
I have heard of others talk about blow's but I think if ya drive them too fast you are going to get the blow if the twist is far too fast.

I've had it happen twice using this combination.

Cartridge: 22-250, 26" 1-14 twist barrel
Powder: Hodgdon Varget, 38.3 grs
Bullet: Molycoated Speer 50 grain TNT, seated far out
Primer: Fed 210M
Velocity: 3900+

I have had failures with this combination. one or two out of five would not reach 100 yards.

Remington 700 BDL factory barrel in:
.17 Remington
Berger 25 grain bullet
Varget at 23.8 grains

Clean or dirty barrel didn't seem to matter.

i had a 6xc 105 berger 35.5 4350 it would get to 200 but not 600 factory tubb bbl. A 6br with a new krieger 7.5 twist 88ld would come a part at 100 but the 90 bt shot 10 shots less than .400 at 100 these were all in a tubb gun

Sorry to disappoint but I didn't document the exact load that blew since it was a single slug out of 100 and I considered it "just one of those things" at the time and I pushed it too hard anyhow. Here's what I can tell you:

60gr Berger FB
.243 Win
IMR 4895 with one of the following loads:
45.6gr
45.8gr
46.0gr
46.2gr

Lapua brass, Fed GM primers. There were 25 rounds loaded up for each load above. All 100 showed at least some cratering in the primers,They were all 4000fps+). -Rod-
 
I appreciate the information and keep it coming. We are going to use this information to assist us in some real world testing.

Lapua 308 case formed into above cartridge

36.5g N135,load is from memory, not at home, on lunch break)
3750+ fps, went as high as 4100 fps that day...

Blue streak to the target at 100 yds starting around 3750 fps some made it, some didn't, and strange whistle noise too.
 
22BRW,22BR improved to 45 degree shoulder)
35.0gn Varget
CCI 450
52g Berger Match
1/8 twist Lilja 3 groove barrel

Chrono read average of 3718fps at 15ft.

8 of 10 rnds failed fairly close to 100yds as the target was peppered with jacket fragments and the core.
