Take a hard look at your shell holder. I had a similar problem and the retaining groove for the case was built up with case lube. Had to use a 90 degree pick tool to scrape the build up out of the shell holder.
The build up was hardley noticeable but it was enough to keep the case from centering in the die and the die was skiving a hair thickness wire of brass off the case above the extractor groove.
I regularly clean my shell holders (Redding Competition) every few hundred rounds with a qtip and alcohol. I just checked and it did have a little bit of brass shavings but I'm not sure it's the cause
