Nope. Not once, About 4,000 rounds through mine.

The stationary ejector gets a little loose and is blamed for ejection problems. Staking them with a punch will tighten them up. I opted to put a shim (.003 probably) in beside it. It tightened it up and is removable.

Misread the op's post. Guess I posted a solution looking for a problem. This solution doesn't answer op's question but does work.

My first B14R had a few ejection problems Staked ejector, no more problems. Second B14R no problems. No extraction problems with both.

Yes I did. I sent it to Begara and they repaired it and shipped it back all under warranty.

Yes, and I did the quick fix with a punch to tighten up the extractor and haven't had a problem since.
