This was a movie set where a gun was being used as a prop. There were multiple people who were considered experts or competent persons whose job is to make sure the prop is ready to be used by the actor. Loading the correct ammunition is the responsibility of the expert or competent person. I don't think you could reasonably expect the actor(s) to check the condition of the ammunition/prop every time the expert hands said actor the prop for each and every scene/take.

And, why couldn't it be expected an actor would check the gun every time it's handed to them? Would you expect an actor to check a gun that was handed to them for a suicide scene?
