When I want to be precise, I measure center to center or approximate- measure inside to inside and outside to outside, add the measurements and divide by 2
 
I "establish" center to center by measuring outside to outside of the two most widely dispersed shots and subtracting 1 caliber - works close enough for any bullet diameter for my work. Just one way to do it.
 
Measure a single bullet strike outside to outside using electronic calipers.

Then with the calipers at that width, set the measurement to 0.

Now using the calipers go the to the outside to outside of the group and you will get the center to center size.
 
I thought it was center to center but looking at other posts I wasn't sure. Somebody commented about covering with a dime and said that a dime was .71 and so was the group but it could not be bigger than .49 and still be covered. I quit reloading and shooting for a while and since I've started again BR 50 and Benchrest are the big thing at my club. I like it as some other old farts have said it is hard to shoot offhand as you get older. I really love this site and have learned a lot from it. Clever idea measuring with a digital caliper.
 
I measure center-to-center. When I'm at the range and want to quick check and compare groups I pick up a variety of pistol cases (i.e. 9mm, .40, .45) to lay over the groups when a dime is too big.
