Unfortunately many things ....! having good technique, good reloading procedures and good equipment are all fundamental. It's hard to be more specific but if you're focussed on the reloading element - first can you add how long you've been reloading, the equipment that you are using and your current reloading steps ?

How tight is the 20 round group and how far is the "flier" outside of the main group.

a 5 shot group at 100 yards is well , you can cover it with a nickel . one ragged hole really . but the flyer will be about a half inch off . most of the time to the left for some reason .

first i resize, trim , clean , prime , weigh charges , seat bullets , .... its always worked great always got one hole groups just always get that 1 or 2 flyers

Once you make sure its not you, and you can't tune it out, you may need a barrel. I had a Bartlien that I put 450 rounds though trying to find a load that would not spit a shot out of the group, I even shipped the whole gun to Bartlien as they didn't believe the barrel was faulty. Cut an inch of the muzzle, rechambered, Still did it. Rebarreled, problem gone.

I had the same exact thing happen with a Kreiger, except I shot 1400 through mine before writing it off. Atleast now I have a Dasher fireform barrel.

cjmac,
IMHO, it could be any of the things listed by the previous posters. When's the last time you annealed your brass? Does each bullet seat "feel" much the same? Did you try running all your cases through a Body Die for uniform bases? BTW, what caliber and make of rifle are we talking about (not that it makes any difference). What kinda of brass are you using and are they all sized and trimmed alike as well as fireformed in this chamber? Nickel sized groups are good for just about any "across the counter" rifle. BTW, have you considered pulling the brass that is a "flier" and measuring it in every detail and see if those differ from all the rest IN ANY WAY? Personally, I'd anneal all the brass evenly and start again. And no offense meant, but you may even get to the point when the load, the rifle, and the setup (what setup are you using as that could easily contribute to fliers) are eliminated as being the problem and then there's only one issue left............In any case good luck and hope you solve the problem. We've all been there at onetime or another and absolutely consistency in every aspect is needed for the dime and below sized groups.

When sorting some bullets from base to ogive some times you'll get two groups or more..when shooting these in there sorted groups can't help but notice different impact location.. I gotta think this could cause a flyer if left unchecked.

Sometimes that can happen if you are using the absolute maximum bullet weight recommended for your barrel twist rate. If so, you might try stepping down in bullet weight one notch and see what happens.
