Quick question on barrel indexing

I've read about 15 pages of forum posts on this and I'm still slightly confused.
So do You indicate the chamber and throat true to each other. Now the muzzle end is spinning a circle out the back spider.
I time the "up" indication to the high point of this muzzle "circle"? ( Bore at muzzle higher than chamber centerline)
As opposed to timing the very slight curve of the last couple inches to direct the bullet upwards at exit ? (Bore at muzzle exactly on chamber center line)

Thank you

Watch the Gordy Gritters video it shows the method he uses, I believe he does index the high point of the muzzle runout to the 12 oclock position. I don't believe he does any measuring of the bore at the muzzle end.
