Have you checked neck clearance? Does a bullet drop freely into the neck of a fired case? Measurement of the neck OD of a fired case vs a loaded case?

.010" difference is way plenty clearance but with that much, I am quite curious as to why the 2 don't drop in. Have those 2 case mouths been flattened or dented? Are all 3 cases of the same brand ?

I don't know if you've made any progress figuring out your issue. If the pressures were high, I would expect to see those ejector hole marks appear to be smeared to one side but I just thought of another possibility. Do you have any lube in your chamber or on your cases when you fire them? If so and pressure is high, the case may be rotating with the bolt upon extraction. I suppose they might be doing that anyway even if lube is not present. It is hard to tell from the pictures but does the area of those circles on the case heads project out from the case head, or is it just a ring that is either stamped into or protruding from the case head?
Was there a visible ridge or burr around the hole? Whenever you do get it sorted out, I'd be interested in knowing what you find.
