Neck turning question(s)?

I have been shooting F-TR Mid-range for a couple of years now. So far I have not turned the necks of my Lapua brass. Wanting to get more accuracy out of my rounds, I am thinking about starting to turn my case necks but have some questions.

1) Do you turn the necks on brand new brass or do you wait to fire form them first.
2) I leave the carbon to build up in the neck for lubrication. Would it be more desirable to wait until this build up before turning? My thought is that the carbon has thickness and it would seem logical that I want to make the neck thickness the same based on the total volume not just the brass.
3) Can you turn old brass that has been fired 6 times, what about 20 times?
4) Are you looking for a specific neck all thickness or are we just turning to ensure that the neck OD is concentric to the neck ID?
5) Do you turn all the necks to the same diameter, or just to true up any irregularities in the neck wall thickness?
6) Do you size the necks before turning or does it matter?
7) Finally, and probably already know the answer to this but, do you only turn the case neck once in the lifetime of the case?

I know you all have probably answered these questions many times, for that, I am sorry. But to be honest, I haven't found the answers yet.

For myself:
1) I turn new brass necks before firing
2) I use graphite to lube necks for turning new brass
3) No problem turning necks on fired cases. I anneal my .308 brass every 4-5 firings. I would consider annealing before turning so as to ease the effort required. For the sake of precision I would remove carbon in the necks before hand.
4) I use bushing sizing dies so turn for specific thickness so that I don't have to adjust my bushing size
5) I turn all to the same thickness as per 4)
6) I size the case before turning. I polish the pilot to fit sized necks.
7) In 20 firings with Lapua Palma .308 brass I have trimmed in total about .040". I have not seen appreciable change in neck wall thickness so do not turn the necks beyond the initial turn.
Bear in mind that there is some flow of brass during firing but in seating bullets have not detected any appreciable increased effort over 20 firings while using the same bushing I started with.
