Looking for Help Identifying Rings and Base

I have been searching the web for quite a while and am unable to identify this rail and rings.

The 13 slot rail is steel, 5 1/2" long, 4 screw, one end is taller than the other, .200 wide slots. Can't remember which rifle it fits or who manufactured it.

The 4 screw rings are not magnetic and have a removable insert. I have not seen any with the milled slot in the front and back as shown.

Any help would be greatly appreciated.
