Is the stem contacting the ogive prior to contacting the tip?
Get your favorite black sharpie and color the tip and the ogive. Let dry and press bullet into stem.
IF the ink is worn off of the tip and not the ogive, easy to fix.
Drill the center of the seating stem 0.030" larger in diameter and 0.100" deeper. Ink the bullet and check if this fixes the problem.
