Hadn't found what I was looking for 'cuz I really didn't know what I was looking for I guess. So I cobbled something together that seems to work for me, so far anyway. I've attached the pdf files for anyone to download, I just ask for feedback if you have a moment. I've no intention of selling anything, no strings attached, just wanted to share, fwiw.
I number the styrofoam inserts in ammo boxes like pins on an IC so I can load four sets of five rounds to test powders or powder loads, thus the groups of four.
Italicized items are options or alternates the idea being to strike through the one,s) not used. I use the note blocks to note settings for a Wilson trimmer or runout or whatnot.
Target and numbered blocks are for marking sun angle for groups or group patterns or both. Main target for cold bore shot or whatever. Scale is noted in block to the side. Loc is location, Rest is bags,'pod & bag, rest and rabbit ears, etc. Range is distance, heading, and time. DA is density altitude.
Printed front and back, cut in half, and 3-hole punched on 5-1/2" centers will fit a 6x9 notebook although I use loose 1" binder rings instead. A pkg of Xerox 67# Vellum is $15 at Office Depot here. Sturdy and takes ink well and at 250 sheets it'll probably last forever. Wanna get fancy, use a corner rounding punch and add some thin plastic covers.
