We ask the same silly questions when doing your admission paperwork.They are prying about ownership and depression as a guise for suicide prevention and thus confiscation as a medical solution. It's constant nowadays.
I think one way to get them from being reported at all or someone failing to interpret a joke which many have said they have made would be to not answer the questions at all. I mean I get it, I do. Its a biopsychosocial model where all of healthcare is encompassed all in one package, mental, physical and social, which is great. True overall wellness shouldn't be defined by the absence of disease it should be about the person as a whole. but his nurse doesn't need to know the color of his underwear, or if someone is the house is beating him. Unless you are clearly an immediate danger to yourself or others (i'm talking terroristic threats, murder threats, abuse) there would be no reason that anything would be reported because this is considered private health information.
The questions may be silly, but I can assure you that they are just trying to help, no tin foil hat needed for this.

Just like "they were just doing their job" during covid and scaring everyone and you are going to die if you don't! Its more like we are going to lose our jobs if we don't do what the government, cdc and who ever else says. Instead they should have pushed back and stood up and said prove to us this is safe and we will push it. Instead they rolled over and the general public has payed the price. Why put words in my mouth? Never said that. However, if a nurse comes into my home and starts asking personal questions about firearms and other sensitive topics I'll show them the door. That's a big difference from a made up scenario that rarely happensnot sure why they asked the color of his underwear, but I think trying to help those that might be being abused is pretty important.
