Handstop

If you keep your sling arm relaxed, shouldn't. The trick is making sure you are not muscling the rifle to center the target. That said you will have less hand pain if you allow the shoulder to take a fair share of recoil instead of your sling arm/hand. My sling is snug but not overly so.
 
Inconsistent cheek pressure, tension in your shoulder or back, muscling the rifle with your support arm can cause vertical.
Handstop or sling pressure could cause problems if it's causing too much pain which can cause tension to build in your shoulders and back.
You should be completely relaxed. Use your NPA (natural point of aim) to do the aiming. If your NPA is off then adjust youself so you don't have to use any muscles to aim.
