donuts or dough nuts

I've been doin some reading here and now I have some questions. What is the best way to avoid donuts? Now that I have them, what is the best way to remove?

To avoid them you need to turn your necks into your neck/shoulder junction,to remove them you need to ream out your necks.

I used to run a tight neck and turned my brass like you said. Didn't have donuts, but then again it was a 6.5x284 shehane and didn't have the barrel long enough to find out. Now I run a 284 shehane no turn. I guess I can run a .0005 cleanup into the shoulder a bit?
Or do I have to get all the way into fresh brass all the way around?
Best way to ream the donut out?

If you use an expander mandrel, it will make the neck ID constant and move the donut to the outside; then you can just turn the neck to remove it. I've found that my necks thicken after a few firings, so I have to turn anyway; better to get two birds with one stone.
