Valve failure caused a lot of damage

How did the valve fail? It is in one piece and shows no cracks in the body. The cheap plastic fitting that connected to the valve looks pretty ragged though.

Looks like the plastic fitting melted due to the metal valve acting as a heat sink.

probably over tightened when assembled. big problem when plastic to metal assembly. I dealt with constantly with the new techs I work with.
