40SW cast bullet ??

non rifle ? I have a Walther ppq in 40 sw, reg rifling, shoot jacked stuff great one ragged hole,

I have tried the RCBS 180 cast bullet sized .401 in a Star sizer, and the thing k-holes!!!! at 15 yds

in my other 40s this has never happened, tried Univ 5.1 gr got 980 fps which is not outlandish,

SAECO hardness tester 6.0 = oal 1.120 win primer

I have shot 1000's cast in ipsc and bullseye - even killed deer with cast from a single action Colt and never had this over a 55 year period.

what is going on??

Are you getting any barrel leading? If so, where is it?
Have you slugged the barrel?
Keyholing: screams "Bullet is too small in diameter"
The GUN tells you what diameter to use, and yours sound like it wants a larger bullet.
Are these commercial cast bullets, and, therefore, probably at 18-22BHN? Bullets that hard really need to be at least 0.001" larger than softer lead.
If casting your own, get the alloy down to 10-13 BHN. Next, DON'T size them at all. I gave up sizing any of my cast bullets back around 1977 or so and never looked back.
Try some LLA or 45/45/10 and see how they do. Place all bullets in glass casserole dish, set bullet on side, squirt or pour a little tumble lube on them and shuffle/rotate them around for 30-60 seconds. Inspect all bullets and ensure they are shiny and wet looking but NOT discolored (NOT amber/brown). Dry over night.
If you can't stand the thought of not sizing the bullets, then get a 0.402 or 0.403" sizing die.
Or, get coated bullets from Precision Bullets of Kemp, TX.
 
Also how much taper crimp are you applying? I've seen excessive crimping size bullets down.

I'd bet you need to go .402" as well as SOFTER.
